The LECOM Institute for Successful Aging offers primary care and consultative clinical services in geriatric medicine across the entire continuum of care. Our specialists work with primary care physicians, patients and families to encourage independence and shared decision-making in outlining the appropriate plan of care. In short, the Institute can diagnosis and treat an acute illness, provide transitional care after surgery, and also facilitate short term and long term rehab all within the same health care campus. The Institute also works closely with the LECOM Orthopedic and Vascular Surgery group and other specialists to ensure optimum treatment and care.
James Y. Lin, D.O., MS (Med Ed), Director
Dr. Lin is a Geriatric Medicine Specialist with MAE and Clinical Professor of geriatric Medicine at the Lake Erie College of Osteopathic Medicine. He received his medical degree from LECOM and completed his residency in Internal Medicine at Millcreek Community Hospital. In addition, he received his fellowship training at the University of Medicine and Dentistry of New Jersey. Dr. Lin is also board certified in Internal Medicine and is capable of providing comprehensive care for seniors in both the outpatient and inpatient setting.
Danielle M. Hansen, D.O., MS (Med Ed)
Dr. Hansen is an Internal Medicine and Geriatrice Medicine Specialist with MAE, Vice-President of Acute Care Services at Millcreek Community Hospital (MCH), Associate Director of the LECOM Institute for Successful Aging and Clinical Assistant Professor of Internal Medicine at the Lake Erie College of Osteopathic Medicine. She received her medical degree from LECOM and completed her internship and residency at Millcreek Community Hospital. She also received her Fellowship in Geriatrics from LECOM. She is the 2008 recipient of the American College of Osteopathic Internists Humanism and Excellence in Teaching Award, and she is past recipient of the Alice J. Coppola Internal Medicine Award.
Douglas Fronzaglia, D.O.,
Dr. Fronzaglia is a staff physician at Millcreek Community Hospital and the LECOM Institute for Advanced Wound Care, specializing in Geriatric Medicine, Preventive Care, and Complex Cases. In practice for 14 years, Dr. Fronzaglia is a Certified Wound Care Specialist and manages patients at multiple nursing homes and assisted living facilities in Erie and Crawford counties.
An Erie native, Dr. Fronzaglia graduated from the University of Pittsburgh in 1996 and earned the Doctor of Osteopathic Medicine (DO) degree from LECOM in 2000. Dr. Fronzaglia completed a residency in internal medicine as well as a fellowship in geriatrics at Millcreek Community Hospital.
Blake Hoppe, D.O., Neurologist
Specializing in treatment of headaches/migraines, peripheral neuropathy, Parkinson disease, and falls, multiple sclerosis, memory loss, epilepsy, and dementia, Dr. Hoppe received her medical degree from the Lake Erie College of Osteopathic Medicine and completed her residency in Neurology at Hamot Medical Center where she also served as Chief Resident at the end of her residency program. In addition to her specialty, Dr. Hoppe is also a Clinical Assistant Professor of Internal Medicine and Neurology at LECOM.
Nannette Crawford, MACCC-SLP
With over 35 years experience in the field of Speech-Language Pathology, Nannette has treated children, adolescents, adults and geriatrics who have communication, cognitive-communicative and swallowing disorders. She specializes in the evaluation and treatment of adolescents, adults and geriatrics. She is PA licensed with a Certificate of Clinical Competence (CCC).
Jason Lee, D.P.M., Podiatry
Dr. Lee received his medical degree from Des Moines University, College of Podiatric Medicine and Surgery and completed his residency program at Millcreek Community Hospital. He is board certified in podiatry. In addition, he is Clinical Assistant professor of Podiatry at the Lake Erie College of OsteopathicMedicine (LECOM).
Susan Evans, Ph.D
Dr. Evans is a licensed Psychologist with the LECOM Institute for Successful Aging. Prior to her role with the Institute, she operated a private practice for sixteen years and spend the last eight years been a part of the North Shore Neuroscience Group. She received her Bachelor’s of Science degree from Penn State, Masters in Clinical Psychology from Edinboro University and her PhD in Counseling and Psychology from Gannon University. In addition, Dr. Evans serves on the Erie County Court of Common Pleas as the Special Master to the parent coordinator program for high conflict families. She is also a member of the American Psychological Association and the Pennsylvania Psychological Association.

Geriatric Assessment Program -The Geriatric Fracture Program is LECOM Health System’s answer to our community’s need for a more efficient way of treating fractures in older adults. This program is based on a list of nationally recognized best practices that uses standardized protocols that significantly increases your quality of life after treatment and decreases your time away from home. 868.7850
Acute Care for the Elderly (ACE unit) -The ACE Unit is a pioneering and unique concept in geriatric healthcare that focuses on all aspects of hospitalized elders while rendering a continuum of exemplary and efficient care. 864.4031
Transitional Care Unit (TCU) -The TCU at Millcreek Community Hospital is licensed as a 24 bed, short-term skilled nursing unit and is designed to help senior patients return to their previous level of independence following surgery or illness that required inpatient hospitalization. 868.2634
Millcreek Manor -The Manor is a 50 bed skilled, long-term care unit located adjacent to Millcreek Community Hospital. The Manor offers rehabilitation, nursing and physician services. The staff ratio is above state requirements enhancing the personal attention and service we can give to our residents. 868.2634
Expanded Mental Health/Geri Psych Services - This unit is the regions largest in-patient behavioral health unit geared specifically for seniors experiencing changes in their behavior and/or demeanor. During their stay, seniors receive a comprehensive medical as well as emotional evaluation and a variety of therapies to provide the stabilization needed. 864.4031
The LECOM Institute for Successful Aging is located on the third floor of the LECOM John M. & Silvia Ferretti Medical Fitness & Wellness Center which is home of several specialty practices of Medical Associates of Erie. This unique location provides opportunity for an interdisciplinary approach with the resources available to provide a full spectrum of services including radiology and physical therapy.
